A slot cancelled at 9am for 2pm is worth filling. The reason it usually is not filled is that the waitlist lives somewhere nobody can act on in five minutes.
Backfill is a speed problem, not a demand problem. Most specialty clinics have more people who want an earlier appointment than they have gaps. What they do not have is a person free to work through a list at the moment the gap appears.
They ask the waitlist "would you like an earlier appointment?" and wait for a reply. Ask instead for a specific slot with a specific hold time. A yes/no question about a real time gets answered; an open invitation gets ignored.
